Personal Security Information
In addition to the enrollment paperwork, a new Coast Guard Auxiliary member needs the following:
- Two fingerprint cards
- OFI 86C (Also known as a SAC form)
- SF-85
- Verification of citizenship with a copy of the document used to verify the citizenship
A member involved in Direct Operations needs:
- Two fingerprint cards (these are required in addition to any fingerprint cards sent in previously)
- SF 86
- DHS 1000-9
- Verification of citizenship with a copy of the document used to verify the citizenship (again, this is required even if this information was forwarded previously)
